| 520 | 2 | _aThe aim of this study was to assess the efficiency of the computer-guided interpositional sandwich osteotomy for the vertical bone gain and effects on vital structures (inferior dental nerve bundle) compared to free hand interpositional osteotomies. A total of 7 patients were randomly chosen for a split mouth study (total of 14 cases) with atrophic alveolar ridge at posterior mandible were recruited from the outpatient clinic of Oral and maxillofacial surgery department, Faculty of oral and dental medicine – Future University in Egypt. The treatment modalities were randomly allocated (from October 2021 to September 2022) patients randomly divided into two groups • Study group: posterior mandible bone augmentation with fully computer guided sandwich osteotomy technique using patient specific cutting guide and a patient specific cutting guide fixation guide. • Control group: posterior mandible bone augmentation using free hand sandwich osteotomy technique. Amount of Bone Gain Both groups were assessed in terms of bone gain. After 4 months, the CBCT used to measure mean amount of bone gain at three-point (crestal, 5 mm below the crest, 10 mm below the crest) and the result showed that the mean value of bone gain of Study group (2.6±1.03) mm was higher than the mean value of control group (1.98±1.35) mm yet the difference was not statistically significant (p=0.357) the mean value of bone gain in percentage of study Neurosensory affection The study group showed better results when it came to the affection of the inferior alveolar bundle with value of (9.15±0.29) compared to the values of the control group (6.75±1.59). these results simply demonstrated a statistical significance (p=0.015) which by other means indicated that the use of patient specific CAD/CAM guides enhanced the safety of the osteotomies done above the mandibular canal. | |
