Since the development of the edgewise appliance and the advent of different alloys in fabricating archwires, orthodontists shifted between using 0.018 inch bracket slot and 0.022 inch bracket slot. Clinical studies that compared the quality of treatment between both bracket slot sizes are scarce making the clinicians’ choice based only on personal preferences. In this study, 0.018” and 0.022” slot sizes were compared in terms of three-dimensional evaluation of tooth position and duration of alignment stage. The tip and torque movements of upper and lower teeth were evaluated before and after alignment and leveling using CBCT images, while duration of alignment was assessed by recording the number of visits. Fourteen patients with mild to moderate crowding having normal skeletal and Angle Class I molar relation were included in the study. The sample was randomly and equally divided into 2 groups where Group 1 was treated with 0.018-inch bracket slot and Group 2 with 0.022-inch bracket slot. A standardized wire sequence was followed in each group where in Group 1 (0.018-inch) bracket slot the wire sequence was 0.014” and 0.016”, 0.016”×0.022” NiTi followed by 0.016”×0.022” and 0.017”×0.025” StSt and for Group 2 (0.022-inch) bracket slot it was 0.014” and 0.016”, 0.016”×0.022” NiTi, followed by 0.016”×0.022”, 0.017”×0.025” and 0.019×0.025 StSt. CBCT images were used to assess pre and post- alignment tip and torque movements accurately and were compared between both groups. CBCT images were taken prior to bonding brackets and one month after reaching the final wire. The investigator assigned for measuring the obtained CBCT images was blinded. Number of visits were recorded for both groups till the end of the alignment phase reaching the last wire. The results obtained from the current study showed: 1. Insignificant difference in the crown inclination and angulation of maxillary and mandibular teeth was found between both groups. 2. A statistically significant shorter duration of alignment was found for Group 1 (.018”) compared to Group 2 (0.022”) slot however, this difference may be clinically insignificant. Within the limitations of the current study, the following can be concluded: 1. None of the bracket slot sizes attained superior results over the other when it came to tip and torque movements in both maxillary and mandibular arches. 2. 0.018 inch bracket slots might have a slightly shorter duration of alignment compared to 0.022” slot brackets.
